月薪港幣120,520元至129,375元(視乎工作經驗而定)
職責
主要職責包括:
(a) 領導地區康健中心組規劃和推行地區康健中心計劃和慢性疾病共同治理先導計劃,有關工作包括處理18個規模完善的地區康健中心/地區康健站的合約管理事宜,籌備把地區康健站過渡至地區康健中心,以及規劃推行慢性疾病共同治理先導計劃;
(b) 為地區康健中心人員提供策略性督導和指導,並為地區康健中心制定發展及服務計劃和工作目標,確保實行適當的人力資源管理、財務管理、預算和基層醫療服務採購;
(c) 監督和評估地區康健中心營運機構的表現;
(d) 與地區康健中心計劃和慢性疾病共同治理先導計劃的持份者,包括專業團體、區議會和大學等聯繫和溝通;
(e) 處理與地區康健中心計劃有關的秘書處工作;以及
(f) 確保地區康健中心的服務與政府各局/部門的基層醫療服務協調和銜接。

The main duties include:
(a) To lead the District Health Centre (DHC) Team to plan and implement the DHC Scheme and Chronic Disease Co-Care (CDCC) Pilot Scheme, including contract management of 18 full-fledged DHCs or 'DHC Expresses', preparation for the migration of 'DHC Expresses' to DHCs as well as planning to launch the CDCC Pilot Scheme;
(b) To provide strategic steer and direction to DHC team members, set development and service plans and work targets, ensure proper human resource management, financial management, budgeting, procurement of primary healthcare services for DHCs;
(c) To supervise and evaluate performance of DHC operators;
(d) To engage and communicate with different stakeholders of the DHC Scheme and CDCC Pilot Scheme, including professional associations, district councils and universities etc.;
(e) To handle the secretariat work related to DHC Scheme; and
(f) To ensure service coordination and interface of DHC services with various primary healthcare services of Government Bureaux / Departments

入職條件
申請人須:
(a) 具備根據香港法例
i. 第138章 《藥劑業及毒藥條例》第3部、
ii. 第164章《護士註冊條例》或
iii. 第359章《輔助醫療業條例》
於香港註冊的資格;
(b) 在取得資格後,具備至少十年醫院或診療所管理/行政的全職工作經驗,以及深厚的高級管理和督導經驗(註1);
(c) 具備組織、指導、規劃和統籌社區健康服務項目的能力;
(d) 具有領導才能,包括與提供服務機構、社區組織及其他有關方面商談、解決問題和建立關係的能力;以及
(e) 具備良好中英文口語及書寫能力。

Candidates should have –
(a) a qualification registrable in Hong Kong under
i. Part 3 of Pharmacy and Poisons Ordinance, Cap. 138,
ii. Nurses Registration Ordinance, Cap. 164 or
iii. Supplementary Medical Professions Ordinance, Cap. 359;
(b) at least 10 years’ full-time post-qualification management/administration experience in hospitals or medical clinics, with solid senior managerial and supervisory experience (Note 1);
(c) demonstrated ability to organize, direct, plan and coordinate community healthcare operations; and
(d) leadership skills including negotiation, problem solving and relationship building with service providers, community groups and other related parties; and
(e) a good command of spoken and written Chinese and English.
