職責
主要職責包括：
(a) 就推行《基層醫療健康藍圖》及政府政策中建議的各項基層醫療健康措施提供專業意見及支援；
(b) 就地區康健中心計劃的發展提供專業意見及支援；
(c) 協調不同持份者參與發展醫社合作；
(d) 就服務標準和指引提供專業意見；以及
(e) 支援地區康健中心計劃及其他基層醫療健康服務營運者的委聘事宜，包括擬備招標文件及服務營運合約，以及進行相關評估和評核工作。

The main duties include:
(a) To provide professional inputs and support the implementation of various primary healthcare initiatives as recommended in the Primary Healthcare Blueprint or included in government policies;
(b) To provide professional inputs and support to the development of the District Health Centre (DHC) Scheme;
(c) To co-ordinate with different stakeholders for the development of medical-socio collaboration;
(d) To provide professional inputs on service standards and guidelines; and
(e) To support the engagement exercise for operators of DHC Scheme and other primary health care services, including the preparation of tender documents and operation service contracts, evaluation and assessment.

入職條件
申請人須：
(a)
(i) 已根據《社會工作者註冊條例》(第505章)的規定，註冊成為註冊社會工作者(第1類)；或
(ii) 持有香港護士管理局頒發的註冊證明書(第I部／第II部)及香港護士管理局頒發的有效執業證明書；或
(iii) 已根據《輔助醫療業條例》(第359章)的規定，在本港註冊成為物理治療師，名列物理治療師註冊名冊第Ia部分，並持有香港物理治療師管理委員會頒發的有效執業證明書；或
(iv) 已根據《輔助醫療業條例》(第359章)的規定，在本港註冊成為職業治療師，名列職業治療師註冊名冊第I部分，並持有香港職業治療師管理委員會頒發的有效執業證明書；或
(v) 已根據《藥劑業及毒藥條例》(第138章)第3部的規定，於香港藥劑業及毒藥管理局註冊，並持有香港藥劑業及毒藥管理局頒發的有效執業證明書。
(b) 在取得資格後，具備五年全職公共醫療工作經驗，尤以基層或社區服務方面為佳；以及
(c) 符合語文能力要求，即在香港中學文憑考試或香港中學會考中國語文科和英國語文科考獲第2級或以上成績[註(3)]，或具同等成績；以及能操流利廣東話和英語。
(a)
(i) 已根據《社會工作者註冊條例》(第505章)的規定，註冊成為註冊社會工作者(第1類)；或

(b) 在取得資格後，具備五年全職公共醫療工作經驗，尤以基層或社區服務方面為佳；以及
(c) 符合語文能力要求，即在香港中學文憑考試或香港中學會考中國語文科和英國語文科考獲第2級或以上成績[註(3)]，或具同等成績；以及能操流利廣東話和英語。
Candidates should have –
(a)
i. registered as a registered social worker (category 1) under the Social Workers Registration Ordinance, Chapter 505; or
ii. a Certificate of Registration (Part I / II) issued by the Nursing Council of Hong Kong and hold a valid practising certificate issued by the Nursing Council of Hong Kong; or
iii. registered in Hong Kong as a Physiotherapist in Part Ia of the Register of Physiotherapists under the Supplementary Medical Professions Ordinance (Cap. 359) and hold a valid practising certificate issued by the Physiotherapists Board of Hong Kong ; or
iv. registered in Hong Kong as an Occupational Therapist in Part I of the Register of Occupational Therapists under the Supplementary Medical Professions Ordinance (Cap.359) and hold a valid practising certificate issued by the Occupational Therapists Board of Hong Kong; or
v. registered with the Pharmacy and Poisons Board of Hong Kong under the Part 3 of Pharmacy and Poisons Ordinance, Chapter 138 and hold a valid practising certificate issued by the Pharmacy and Poisons Board of Hong Kong;
(b) five-year full-time post-qualification work experience in the area of public healthcare, preferable in the field of primary or community service; and
(c) met the language proficiency requirements of Level 2 or above in Chinese Language and English Language in the Hong Kong Diploma of Secondary Education Examination or the Hong Kong Certificate of Education Examination (HKCEE)[Note 3], or equivalent; and be able to speak fluent Cantonese and English.
(a)
