A組：

擔任該職位的人員負責在地區健康服務制度下協助發展醫社協作網絡；協助制定地區康健中心的社會服務標準及指引；建立全面的社區醫社服務(包括公營／私營／非政府機構)資源資料庫；建立外展機制及策略，讓市民獲得更便捷的地區康健中心服務；以及就地區康健中心營運者的醫社協作服務提供意見。

B組：

擔任該職位的人員負責推動地區康健中心和地區康健站的持續發展工作，並帶領團隊確保地區康健中心和地區康健站有令人滿意的表現；整理由基層醫療健康辦事處轄下其他組別所提供的資料和專業意見作地區康健中心的新服務發展；就服務標準和指引提供意見；就地區康健中心營運服務合約的招標文件和地區康健站的建議書的擬備工作提供意見，並協助評審投標書和地區康健站的建議書；監督事故報告的檢視，處理地區康健中心／地區康健站的營運者就處理事故所要求的協助，以及在有需要時在決策局層面處理投訴並進行調查。

Stream A:

The incumbent is responsible for assisting in developing a medical social collaboration network under a district health system, developing the social service standards and guidelines for District Health Centres (DHCs), developing a comprehensive community medical social service (including public / private / NGOs’) resource database, developing the outreaching mechanism and strategy to enhance accessibility by target clients to DHC services and providing advice on the medical social collaboration service of the DHC operator.

Stream B:

The incumbent is responsible for engaging in the continuous development of DHCs and DHC Express and leading the team to ensure satisfactory performance of DHCs and DHC Express, collating information and professional inputs from other teams under the PHO for the development of new DHC services, providing advice on the service standards and guidelines, providing inputs to the preparation of tender documents of DHC operation service contracts and DHC Express proposals and assisting in the assessment of tenders and DHC Express proposals, overseeing the review of incident reports and providing response to requests by DHC/ DHC Express operators for support on incident handling and handling complaints at bureau level when required and conduct investigations.
